Support That Meets You Where You Are
Community care is about connection. Our peer specialists and community health workers walk alongside you to find resources, navigate services, and build the support network you need to stay well, on your terms.
Our team can help with housing navigation, benefits support, reentry assistance, and planning your next steps together. Getting started is simple: complete a short intake, we review it and match you with services, and then we help you take your next step.

Built Around the People We Serve
Our community care team works with individuals and families, justice-involved individuals, women, and Veterans. Everyone brings their own story, and our support is shaped around where you are today and where you want to go.
Whether you are returning home after incarceration, supporting your family, or looking for help as a Veteran, our peer specialists and community health workers offer lived experience and practical guidance. Together, we help you find resources, navigate systems, and build lasting stability.

Making Sense of Your Benefits
Health coverage can feel confusing, especially during a time of change. Our team can help you verify your coverage, understand your benefits, and get referrals, so you can focus on your care instead of the paperwork.
We can explain what your plan covers, help with referrals, and stay with you for ongoing support as your needs change. We also help with other essential benefits such as SNAP and SSI/SSDI, so you can keep moving forward with less stress.

The Plans We Work With
We work with Medi-Cal, LA County DMH, and several major health plans. If you are not sure whether your plan is included, reach out and our team will help you check.
These include L.A. Care, Health Net, and Kaiser Permanente, along with partners such as CalMHSA and Optum. Coverage and eligibility vary by plan, so our team will help you confirm your options before you begin.
